“You’ve been told it’s a rotator cuff tear, but your MRI came back clean. Yet, every time you reach into the backseat of your car or try to tuck in your shirt, your shoulder feels like it’s slipping, followed by a sharp, biting pain in the front.”
This is the frustration of the “clinical ghost.” In many Ontario primary care settings, if a shoulder hurts and the X-ray is normal, it’s labeled “impingement.” If the patient is over 50, it’s “biceps tendonitis.” But for a significant cohort of patients, the standard diagnostic net is casting too wide, missing a specialized structure that acts as the “linchpin” of the anterior shoulder: The Rotator Interval.
The ESP Insight: The “Sixth Sense” for Stability
As Extended Scope Practitioners (ESPs), our “Sixth Sense” isn’t magic—it’s the result of performing thousands of specialized physical exams where we look between the standard diagnoses.
The Rotator Interval is a triangular space between the superior border of the subscapularis and the anterior border of the supraspinatus. It houses the long head of the biceps tendon and the superior glenohumeral ligament (SGHL). While a full-thickness rotator cuff tear is easy to spot on imaging, a Rotator Interval Sprain or “micro-instability” of this interval is notoriously subtle.
Standard MRIs often fail to show the loss of tension in the coracohumeral ligament. However, during an ESP assessment, we look for the “Sulcus Sign” in external rotation or the “Passive Extension Test.” When this interval is lax or sprained, the humeral head shifts slightly forward and upward—not enough to dislocate, but enough to create a “micro-instability” that mimics the pain of a tear.
The “Why it Matters”: Why Standard Physio Might Be Failing You
If you have a Rotator Interval sprain but are being treated for “biceps tendonitis,” your therapist might be focusing on eccentric loading of the biceps. However, if the interval is lax, the biceps is actually working overtime to compensate for the lack of ligamentous support. Loading it further can actually increase the anterior shear of the shoulder.
Mechanically, the Rotator Interval acts as a “dam” against the humeral head shifting upward. When it’s compromised:
- Proprioceptive Deficit: The brain loses track of exactly where the joint is centered.
- Coracoacromial Arch Irritation: The subtle shift leads to secondary “pseudo-impingement.”
- The Surgical Pivot: Unlike a degenerative cuff tear which may respond well to gradual loading, a true Rotator Interval insufficiency in an active patient may require a surgical “closure” to restore the suction-cup effect of the joint.
